Primary Function

The primary function of 24/7 Team Care Home Health is to provide Skilled Nursing, Home Health Aide, and Allied Therapeutic Services to home-bound patients on a part-time, intermittent basis in a place of residence used as the indivdual’s home under a plan of treatment established by the patient’s physician.

Goals

Patients will be maintained in the home as long as feasible through the provision of intermittent skilled nursing care, the multi-disciplinary team, and physician follow-up.

Patients will be helped to function to the maximum level of their ability: physically, emotionally, and socially.
